Volunteer Hours are an important part of this organization. Hours are recorded from November 1st thru October 31st.
You can read how to fill out the Volunteer Hour Report form by going to this two-page Instructions for Volunteer Hours Report sheet.
The Appropriate Codes sheet contains the Agency Code and District required on the Volunteer Hour Report form.
Recommend you print these two forms out to refer to as necessary.

Then, just print off your form and start recording your hours!

This Volunteer Hour Report form is in Excel on the State BCHW page. Click on the Volunteer Hour link above. The line - "Click here for a report form" - on the state page appears. When Open, Save or Cancel block appears, choose Save and send to your own computer to use from there.
Be sure to turn them in to the Volunteer Hour Coordinator often - monthly is preferred.
The last date to turn them in to count is November 15th.
